Overview
The New Claim form includes a Loss Information area where you record the date and type of loss, apply visual color highlights for quick identification on the Manage Claims grid, set claim-level flags for special handling, and classify the claim using industry-standard statutory codes. This article covers all of those fields.
Date and Type of Loss
Date of Loss — Select the date the loss event occurred using the date picker. This date drives milestone calculations, statutory deadlines, and reporting timelines throughout the claim's life.
Type of Loss — Select the peril from the dropdown (for example, Hail, Flood, Wind, Fire, Water Damage). This classification determines which fields are relevant on the rest of the form and may trigger carrier-specific workflows such as TORRENT reporting for flood claims.
Urgency and Special Flags
Total Loss — Urgent Contact — Check this box when the claim involves a total loss that requires immediate contact with the insured. This flag alerts the claims team that the situation is time-sensitive.
MOLD Claim — Check this box if mold is present or suspected. Mold claims often have specialized handling requirements, reporting obligations, and remediation protocols that differ from standard property claims.
Color Highlights
Evolve provides six color-coded checkboxes that you can apply to any claim. When a color is checked, the claim row on the Manage Claims grid displays that color, making it easy to spot specific claims at a glance.
The available colors are: BLUE, PINK, GREEN, VIOLET, ORANGE, and GRAY.
You can check more than one color on a single claim. The meaning of each color is defined by your organization — Evolve does not assign built-in meanings. Common uses include marking priority levels, catastrophe events, litigation holds, or claims requiring special attention. Your administrator can configure the meaning of each color in Settings under Color Coding for Reports and Documents.
Classification Fields
Unit — Select the unit type from the dropdown (for example, Residential, Commercial). This categorizes the claim by property use.
Type of Adjustment — Select the adjustment type from the dropdown (for example, Limited, Full, Supplemental). This describes the scope of the adjuster's assignment on the claim.
Statutory Codes
The statutory code fields classify the claim according to industry-standard coding systems. Each field opens a "Click to Select" lookup that displays available codes and their descriptions.
Statutory LOB Code — The Line of Business code that identifies the insurance line under which the claim falls (for example, Homeowners, Commercial Property, Inland Marine). LOB codes are used for regulatory reporting and carrier-level analytics.
ISO Code — The Insurance Services Office code that classifies the claim according to ISO's standardized coding scheme. ISO codes enable consistent data comparison across carriers and are required by many state regulators.
Claim Cause Code — A code that identifies the specific cause of the loss in more detail than the Type of Loss dropdown. Cause codes provide granular classification for reporting (for example, distinguishing between wind-driven rain and straight-line wind).
% Ceded — Select the percentage of the claim that is ceded to a reinsurer. This field is relevant for claims under treaties or excess-of-loss reinsurance agreements. If the claim is fully retained, leave this field empty or at zero.
RISK Code — Select the risk classification code from the dropdown. The RISK code categorizes the type of risk insured and is used in underwriting analytics and loss-ratio reporting.
Additional Loss Information
PCS Code — Select the Property Claim Services catastrophe code if the loss is part of a declared catastrophe event. PCS codes are assigned by Verisk and allow carriers to aggregate claims by catastrophe.
CAT Code — Select your organization's internal catastrophe code, if applicable. This may differ from the PCS code and is used for internal tracking and resource deployment.
Loss Description — Enter a free-text description of the loss event. The My Clippings dropdown above the text area lets you insert pre-written text snippets to speed up data entry. Clippings are configured by your administrator and can include standard language for common loss scenarios.
Tips
Color highlights are one of the fastest ways to visually manage a large claim portfolio. Agree on color meanings across your team so everyone interprets the Manage Claims grid consistently.
Statutory codes are often required for carrier reporting. If you are unsure which code to select, check with your manager or refer to the carrier's reporting guidelines.
The My Clippings feature in the Loss Description field can save significant time on repetitive claim types. Ask your administrator to add clippings for your most common scenarios.
Multiple color highlights on a single claim can indicate overlapping conditions (for example, a claim that is both high-priority and in litigation).
